Introduction These Terms and Conditions (“Terms”) govern your use of the Breakthroughs LLC website and related online services (the “Website”). “We,” “us,” or “Breakthroughs” means Breakthroughs LLC, a home care agency for adults with disabilities. “You” means any visitor, client, family member, guardian, caregiver, or staff member who accesses or uses the Website. By using the Website you agree to these Terms.

    Scope of Use

  • The Website provides information, account access, care coordination features, scheduling, documentation, training resources, communication tools, and related services (collectively, “Services”).

  • Use of the Website is permitted only for lawful, authorized purposes related to receiving or delivering home care services or managing care, training, or employment with Breakthroughs.

  • You must be 18 years or older or have legal authority to act on behalf of a client to create an account or enter into agreements here.

Accounts, Access, and Credentials

  • Accounts: Some Services require an account. You agree to provide accurate information and keep account credentials confidential.

  • Authorization: Employees, contractors, clients, guardians, or authorized representatives may be issued different access levels. Use only the access assigned to you.

  • Security: Notify Breakthroughs immediately if you suspect unauthorized access or loss of credentials.

  • Termination or Suspension: Breakthroughs may suspend or terminate accounts for violation of these Terms, suspected misuse, or for business or security reasons.

Client Care & Service Information

  • Clinical/use information: The Website may host care plans, schedules, progress notes, medication records, and other health- or service-related content. Such information is provided to support, not replace, professional judgment.
