Breakthroughs LLC is a dedicated Medicaid Waiver agency proudly serving individuals and families across Florida through the HCBS (Home and Community-Based Services) Waiver program. Since 2014, our mission has been to provide compassionate, personalized support that empowers adults with intellectual disabilities to live meaningful, independent, and fully enriched lives.
We offer a comprehensive range of services including Personal Supports, Respite Care, Supported Living Coaching, Supported Employment, and Companion Services. In addition, we specialize in transitional services, helping individuals move from group homes or family homes into supported living environments where they can thrive with greater independence and confidence.
At the heart of everything we do is a commitment to easing the burden on family caregivers while ensuring each individual receives care tailored to their unique needs, goals, and abilities. We believe that support should extend beyond daily care—so we also host community events and social gatherings that foster connection, inclusion, and a true sense of belonging.
